Assurance of Confidentiality

The Ohio State University’s Institutional Review Board (IRB) for the Protection of Humans in Research has approved this research. If you have any questions or concerns about this approval you may call the IRB at 1-800-678-6251. The Institutional Review Board is charged with protecting the rights and welfare of human research participants recruited to participate in research activities conducted at the University. The membership is composed of scientists, non-scientists and non-affiliated members of the University and community who serve a three-year term or longer. The IRB meets monthly to review applications requiring approval. All ongoing research projects are reviewed yearly, depending upon the level of risk.

Study records that identify you will be kept confidential as required by law. Federal Privacy Regulations provide safeguards for privacy, security, and authorized access. You will not be identified by name, social security number, address, telephone number or any other direct personal identifier in study records disclosed outside of the Ohio State University. The information obtained in this study may be published in scientific journals or presented at scientific meetings, but your child’s identity will not be indicated.
